Welcome to the spring issue of AUPO FCC's quarterly eCommunication.


Fellow Exit Surveys 2013

With less than a quarter of the year left in the 2012/2013 academic year, Fellows should be fully engaged in your program and their learning process. This is a good time to review compliance requirements to make adjustments, if necessary, or to validate that your program is achieving its goals related to fellow education.

Reminder: The exit survey is a 3 part survey and all 3 sections must be complete in order for the survey to be considered done. The deadline to complete exit surveys is at the end of your fellowship program between June 30th and July 7th.

Extenuating circumstances that might prevent a fellow from completing their exit survey by the deadline must be addressed no later than June 1st; please contact Michael Paulos, Program Manager, at mpaulos@aao.org or (415) 561-8580.

Surgical / Procedure / Case Logs

Fellows are able to print a copy of their surgical / procedure / case logs. Now is a good time to ask them for a copy to view prior to their fellowship end date.

We encourage you to view these logs prior to your fellow(s) graduating to be sure your program is maintaining compliance based on your subspecialty program requirements.

Change in Fellowship Director
 
A change in Fellowship Director can be reported at any time by submitting a letter from the Department Chair advising us about the change. Depending on your subspecialty this should be done 30 to 90 days in advance. Please review your program requirements to ensure the incoming Fellowship Director meets the qualifications.

Follow up correspondence may require completing a biographical sketch form for review by the AUPO FCC Review Committee for continued compliance.  

Please provide contact information including the email address of the new fellowship director. The biographical sketch form can be located on the AUPO FCC website, pick your subspecialty and click on bio sketch form and submit to aupofcc@aao.org.

Annual Fee

Invoices will go out in April and payment is due by June 1. Payment is required for continued compliance.

Triennial / Quadrennial Review

If you are scheduled for a triennial / quadrennial review in 2013 you will receive a notice and instructions by email in May. This is a reminder that the reviews are due on August 15th. In conjunction with this review you will also need to review and approve your 2013 graduating fellow's exit survey in order to completely finish.

Review a sample triennial review online: click on fellowship, choose your subspecialty, and click on sample review.

Request for Increase in Fellowship Positions

All requests for increases in the number of compliant fellowship positions must be received by May 1, 2013 to be eligible for posting for the 2014 available positions.

Increases in AUPO FCC Compliant positions may be requested by the Fellowship Director by May 1 for a July 1 start date via letter or email. Along with the request please provide written approval from the parent institution department chair. The request will be presented to the Review Committee and approval may be based on the submission of additional information at the discretion of the Review Committee. The review process is approximately 30 days.
