Fellow Survey Instructions & Notes
If you have not started your exit survey, use the Create Fellow Exit Survey Login to create your account. Once you have created your account and start your exit survey, use the Fellow Exit Survey Login for your subsequent logins.
Your Fellow Exit Survey is divided into 3 parts and must be completed in sequential and chronological order of your Fellowship. Each part must be completed to move on to the next part.
Part I - Contact Information: We ask for your basic contact and program information. This section must be completed first at the beginning of your fellowship.
Part II - Documentation: Intended to assist you in the collection of the large volume of supporting documentation throughout your Fellowship. Part III - Questionnaire: The exit questionnaire consists of mostly yes/no questions that requires you to evaluate the content and quality of your Fellowship training experience. Part III will be available to you only after you have completed all of Part II and when you are within 30 days of the end of your fellowship (normally June 1st).
