Fellow Survey Instructions & Notes
If you have not started your exit survey, use the Create Fellow Exit Survey Login to create your account. Once you have created your account and start your exit survey, use the Fellow Exit Survey Login for your subsequent logins.
Your Fellow Exit Survey is divided into 3 parts and must be completed in sequential and chronological order of your Fellowship. Each part must be completed to move on to the next part.
Part I - Contact Information: We ask for your basic contact and program information. This section must be completed first at the beginning of your fellowship.
Part II - Documentation: Intended to assist you in the collection of the large volume of supporting documentation throughout your Fellowship.* Part III - Questionnaire: The exit questionnaire consists of mostly yes/no questions that requires you to evaluate the content and quality of your Fellowship training experience. Part III will be available to you only after you have completed all of Part II and when you are within 30 days of the end of your fellowship (normally June 1st).

In collaboration with the American Society of Retina Specialists (ASRS), fellows have the option to upload their Educational Activity log files (Papers/Presentations, Journal Clubs/Conferences) and Surgical Procedure totals to their Fellow Exit Survey. Refer to the ASRS web site for instructions to export your log report file to your personal device.
If you choose to upload your ASRS logs, make sure ALL AUPO FCC required information is entered in the ASRS system. If missing fields are indicated in your file that is AUPO FCC required, your upload will not be accepted. You may return to the ASRS web site to edit your logs, regenerate your report file and try to upload it again. You will also have the option to enter your logs manually without the upload in the AUPO FCC system.
The AUPO FCC Exit Surveys are due by June 30th. Please plan on uploading your file or manually enter your data as close to the end of your fellowship as possible. Please note that you cannot proceed to Part 3 (the questionnaire) until you have finalized the Part 2 logging portion of your exit survey.
Once you have successfully uploaded your log file, you may submit additional entries, but you may not edit the uploaded entries. If 0 (zero) entries are indicated, you will be asked to acknowledge that there is nothing to report. Note that the counts for surgical procedures CAN be edited until you have closed and finalized the Part 2 logging portion of the Exit Survey.
After you have either entered all your logging information or successfully uploaded your logs from the ASRS system, you may print a report (pdf format) of your logs. Make sure to print/save the report before you click on the "Finalize and Close my Part 2 logs" link. Once you have Finalized and Closed your Part 2 logs, you will NOT have access to them.
Remember you MUST click on the "Finalize and Closed my Part 2 logs" link on your Exit Survey home page to have access to the Part 3 questionnaire. All parts must be complete before submitting your required Exit Survey.
Don't forget to create a PDF report of your logs PRIOR to Finalizing / Closing your Part 2 log section.
